Faith Formation Sacramental Preparation PRE-REGISTRATION: Reminder that all children in a Year 1 or Year 2 in a Sacramental Preparation Class must have their family status verified in order to register in Faith Formation. The form to fill out to confirm status is below!  This is required for students in both Year 1 and Year 2. Please list any of your children that will be in Year 1 or Year 2 of Sacramental Preparation (typically 1st/2nd grade, or 7th & 8th grade, but maybe different years if your child needs to prepare for a Sacrament at a different age).
